Olles vaevu pääsenud maa-alusest surmalabürindist, paisatakse Alice (Milla Jovovich) tagasi võitlusesse verejanuliste zombide vastu. Karantiinis linnas liitub ta eliitsõdurite rühmaga, mida juhivad Valentine (Sienna Guillory) ja Carlos (Oded Fehr). Rühma ülesanne: leida üles märatseva T-viiruse looja dr. Ashfordi kadunud tütar.
